Indications

Chemical Composition

5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)

Pharmacokinetic Property

Linear, highly protein bound, largely metabolized in the liver

Mode of Action

Inhibits synthesis of prostaglandins, acts as a peripherally acting analgesic, minimal anti-inflammatory effect at analgesic dose

Tablet Dosage

Recommended dose is 10 mg every 4-6 hours, short-term use only (up to 7 days)

Injection Dosage

  • IV bolus to be given over no less than 15 seconds
  • IM administration should be given slowly and deeply into the muscle
  • Onset of analgesic effect within 30 minutes, maximum effect in 1 to 2 hours after dosing IV or IM
  • Duration of analgesic effect is usually 4 to 6 hours
  • Different doses for different age groups and conditions for single and multiple dose treatments
  • Total combined daily dose of ketorolac (oral + parenteral) should not exceed 120 mg in younger adult patients or 60 mg in elderly patients on the day the change of formulation is made
  • Total duration of combined parenteral and oral treatment should not exceed 5 days

Eye Drops Dosage

Adults: 1 drop in each eye 4 times daily

Drug Interactions

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with Beta Blockers
  • Increased risk of renal impairment with ACE Inhibitors
  • Enhanced toxicity of methotrexate

Contraindications

  • Hypersensitivity to ketorolac or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic before surgery

Pregnancy and Lactation

Category C drug, should be avoided unless benefits outweigh risks

Precautions and Warnings

  • Exercise caution in patients over the age of 65 years
  • Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
  • Patients with evidence of corneal epithelial breakdown should discontinue use of topical NSAIDs and be closely monitored for corneal health
  • Eye Drops should not be administered while wearing contact lenses
