Indications
- Management of moderate to severe acute post-operative pain
- Relief of pain, inflammation, and light sensitivity after ocular surgery
- Reduction of eye inflammation or redness caused by seasonal allergies
Description
- Belongs to the pyrrolo-pyrrole group of NSAIDs
- Chemical name: 5-benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
- Inhibits synthesis of prostaglandins
- Considered a peripherally acting analgesic
- Biological activity associated with the S-form
- Linear pharmacokinetic property
- Highly protein bound
- Metabolized in the liver
- Metabolites and some unchanged drugs excreted in the urine
Mode Of Action
- Inhibits the cyclooxygenase enzyme system
- Minimal anti-inflammatory effect at analgesic dose
Dosage & Administration
- Tablet recommended dose: 10 mg every 4-6 hours, short-term use only
- Injection for moderately severe acute pain management
- IV bolus administration over no less than 15 seconds
- IM administration should be given slowly and deeply into the muscle
- Maximum analgesic effect within 30 minutes to 2 hours after dosing
- Duration of analgesic effect is usually 4 to 6 hours
- Different dosing regimens based on age, weight, and route of administration
- Eye Drops: 1 drop in each eye 4 times daily
Interaction
- Increased side effects with other NSAIDs or aspirin
- Enhanced anti-coagulant effect with anti-coagulants
- Reduced anti-hypertensive effect with Beta Blockers
- Increased risk of renal impairment with ACE Inhibitors
- Enhanced toxicity of methotrexate
Contraindications
- Hypersensitivity to Ketorolac or other NSAIDs
- Use not recommended in children under 16 years of age
- Contraindicated as prophylactic analgesic before surgery

Precautions & Warnings
- Caution in patients over 65 years of age
- Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, or liver dysfunction
- Discontinue use of topical NSAIDs in patients with evidence of corneal epithelial breakdown, monitor for corneal health
- Avoid administration of ophthalmic solution while wearing contact lenses
