Description

  • Dopamine antagonist medication for treating nausea and vomiting

Dosage & Administration

  • Should be taken 15-30 minutes before meals
  • Adults: 10-20 mg every 6-8 hours daily
  • Maximum dose is 80 mg daily

  • Dyspeptic symptom complex
  • Acute nausea and vomiting
  • Parkinson's disease
  • Radiological studies

Mechanism of Action

  • Blocks dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
  • Restores normal motility and tone of the upper gastrointestinal tract
  • Facilitates gastric emptying

Interaction

  • Antagonized by anticholinergic drugs
  • Lowered oral bioavailability when given with antacids and antisecretory drugs
  • Metabolized through CYP3A4 pathway

Contraindications

  • Hypersensitivity to domperidone
  • Neonates
  • Gastrointestinal hemorrhage, mechanical obstruction or perforation
  • Prolactin releasing pituitary tumor

Side Effects

  • Rare transient intestinal cramps
  • Rare extrapyramidal phenomena
  • Possible increase in plasma prolactin level
  • Rare allergic reactions

Pregnancy & Lactation

  • Use during first trimester justified by therapeutic benefit
  • Excreted in breast milk but unknown harm to newborns

Precautions & Warnings

  • Caution in children due to risk of extra-pyramidal reactions
  • Use with caution in hepatic impairment

  • Caution in infants due to immature blood-brain barrier
  • Caution in hepatic impairment
  • Dosage adjustment in severe renal insufficiency

Overdose Effects

  • Drowsiness, disorientation, extrapyramidal reactions
  • Activated charcoal recommended
  • Anticholinergic drugs may help in controlling reactions
