Description

Ketolab is a drug of pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID). Chemically it is known as 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1). Ketolab inhibits synthesis of prostaglandins and may be considered as a peripherally acting analgesic. The biological activity of Ketolab is associated with the S-form. Pharmacokinetic property of Ketolab is linear. It is highly protein bound and is largely metabolized in the liver. The products of metabolism and some unchanged drugs are excreted in the urine.

Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Decrease in eye inflammation or redness caused by seasonal allergies

Pharmacology

Ketorolac Tromethamine is a potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs). It acts by inhibiting the cyclooxygenase enzyme system and hence inhibits the prostaglandin synthesis. It demonstrates a minimal anti-inflammatory effect at its analgesic dose.

Dosage and Administration

  • Tablet: Recommended dose is 10 mg every 4-6 hours for up to 7 days
  • Injection: Single or multiple doses for moderately severe, acute pain
  • Eye Drops: 1 drop in each eye 4 times daily

Interactions

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE inhibitors
  • Enhanced toxicity of methotrexate with methotrexate

Contraindications

  • Hypersensitivity to Ketorolac or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic before surgery

Precautions and Warnings

  • Exercising caution in patients over the age of 65 years
  • Caution in patients with active or suspected peptic ulcer or gastrointestinal bleeding, asthma, and liver dysfunction
  • Discontinue use of topical NSAIDs and closely monitor for corneal health in patients with corneal epithelial breakdown
  • Avoid administration while wearing contact lenses for Lopadol ophthalmic solution
