Indications

  • Short-term management of moderate to severe acute post-operative pain
  • Relieving pain, inflammation, and light sensitivity after ocular surgery
  • Decreasing eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to the pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAID)
  • Chemical structure: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2-amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
  • Inhibits synthesis of prostaglandins
  • The biological activity is associated with the S-form
  • Pharmacokinetic properties: linear, highly protein-bound, largely metabolized in liver, excretion in urine

Pharmacology

  • Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
  • Acts by inhibiting the cyclooxygenase enzyme system and inhibits prostaglandin synthesis
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet recommended dose: 10 mg every 4-6 hours for short-term use (up to 7 days)
  • Injection for moderate to severe acute pain management, with specific dosing based on age and condition
  • Eye Drops: 1 drop in each eye 4 times daily

Interaction

  • Increased side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE inhibitors
  • Enhanced toxicity of methotrexate

Contraindications

  • Hypersensitivity to ketorolac or other NSAIDs
  • Children under 16 years of age
  • Prophylactic analgesic use before surgery

  • Nausea, vomiting, gastrointestinal bleeding, melena, peptic ulcer, pancreatitis, anxiety, drowsiness, headache, excessive thirst, fatigue, bradycardia, hypertension, palpitation, chest pain, infertility in females, pulmonary edema

  • Avoid use in pregnancy and lactation unless benefits outweigh risks

Precautions & Warnings

  • Caution in patients over 65 years of age
  • Caution in patients with active or suspected peptic ulcer or gastrointestinal bleeding, asthma, and liver dysfunction
  • Discontinue use of topical NSAIDs and monitor corneal health in patients with evidence of corneal epithelial breakdown
  • Avoid administration while wearing contact lenses for ophthalmic solution
