Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
- Gastroprokinetic action
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Increases lower esophageal sphincter pressure
Dosage & Administration
- To be taken 15-30 minutes before meals
- Adults: 10-20 mg every 6-8 hours daily
- Children: 2-4 ml suspension/10 kg body weight
- Acute and sub-acute conditions: 20 mg every 6-8 hours daily
- Rectal administration for adults and children
- Maximum dose of 80 mg daily
Interaction
- Anticholinergic drugs antagonize antidyspeptic effect
- Lower oral bioavailability with antacids and antisecretory drugs
- Main metabolic pathway through CYP3A4
- In vitro data suggests drug interactions with CYP3A4 inhibitors
- Does not influence the action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity
- Neonates
- Gastrointestinal stimulation might be dangerous
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Increase in plasma prolactin level
- Rare allergic reactions like rash and urticaria
Pregnancy & Lactation
- Use during first trimester if justified by therapeutic benefit
- No evidence of increase in risk of malformations
- Excreted in breast milk
- Concentrations in breast milk lower than plasma
- Effects on driving and machine use
Precautions & Warnings
- Increased risk of extra-pyramidal reactions in young children
- Highly metabolized in the liver
- Caution in hepatic impairment
