Indications
- Facilitate endotracheal intubation
- Aid in mechanical ventilation
- Assist in a wide range of surgical and obstetric procedures
Pharmacology
- Depolarizing neuromuscular blocking agent
- Imitates the action of acetylcholine at the neuromuscular junction
- Acts on muscle type nicotinic receptors
- Results in opening of the receptor's nicotinic sodium channel
- Leads to calcium release from the sarcoplasmic reticulum
- Results in muscle relaxant effects
Dosage & Administration
- Administered by bolus Intravenous or Intramuscular injection
- Adult dosage dependent on body weight and response
- Children more resistant compared to adults
- Total dose given should not exceed 500 mg per hour
- Administered as intravenous infusion for prolonged surgical procedures
Interaction
- Certain drugs may prolong the neuromuscular blocking effects
- Drugs include specific anticholinesterase agents, cytotoxic compounds, psychiatric drugs, anesthetic agents and drugs
Contraindications
- Not to be administered to patients who are not fully anesthetized
- Contraindicated in patients known to have an inherited atypical plasma cholinesterase activity
- Contraindicated in patients recovering from major trauma or severe burns
- Avoid use in patients with pre-existing hyperkalemia
- Not to be used in the presence of open eye injuries
- Contraindicated in newborn infants
Side Effects
- Cardiovascular effects include bradycardia, tachycardia, hypertension, hypotension, arrhythmias
- Respiratory effects include bronchospasm, prolonged respiratory depression and apnea
- Musculoskeletal effects include muscle fasciculation, post-operative muscle pains, myoglobinemia
- Other effects include hyperthermia, increased intra-ocular pressure, increased intra-gastric pressure, rash, excessive salivation
Pregnancy & Lactation
- Should not be administered to pregnant women unless potential benefit outweighs possible hazards
Precautions & Warnings
- Should be administered by or under close supervision of an anesthetist familiar with its action
- Elderly may be more susceptible to cardiac arrhythmias
