Indications
- Treatment of urinary tract infection
- Effective against Gram-negative organisms
- Targets Proteus species, Klebsiella species, Enterobacter species, and Escherichia coli
Pharmacology
- Synthetic narrow spectrum antibacterial
- Bacteriostatic or bactericidal depending on concentration
- Inhibits bacterial DNA synthesis
- Rapid and complete absorption from GI Tract
- Distributed to most tissues, especially to the kidney and urine
- Short half-life during normal renal function, longer half-life with impaired renal function
- Rapid and complete excretion within 24 hours
Dosage & Administration
- Adult initial dose: 1g every 6 hours for 7 days, then reducing to 500mg every 6 hours
- Children's initial dose: 13.75mg/kg body weight every six hours for one or two weeks
- Children's maintenance dose: 8.25mg/kg body weight every six hours or as prescribed by physician
Interaction
- Contraindicated with melphalan due to possible severe blood containing diarrhea
- Probenecid may elevate serum concentration, enhancing toxicity
- Chlorpromazine and Perphenazine may potentiate the effect of Nalidex
Contraindications
- Infants under 3 months
- Epilepsy
- CNS lesions

Pregnancy & Lactation
- Possibility of causing cartilage and DNA damage
- Excreted in breast milk
- Report of hemolytic anemia in breastfed child of azotemic mother
Precautions & Warnings
- Risk-benefit must be considered during first trimester of pregnancy and breastfeeding
- Caution in impaired renal or hepatic function
