Indications
- Treatment of urinary tract infection
- Effective against Gram-negative organisms
- Targets Proteus, Klebsiella, Enterobacter, Escherichia coli
Pharmacology
- Narrow spectrum antibacterial
- Bacteriostatic or bactericidal based on concentration
- Inhibits bacterial DNA synthesis
- Rapid and complete absorption from the G.I. tract
- Distributed to most tissues, especially the kidney and urine
- Short half-life during normal renal function
Dosage & Administration
- Adult dosage: 1g every 6 hours initially, then 500mg every 6 hours
- Children dosage: Oral administration based on body weight
- Initial and maintenance dosages for different age groups
Interaction
- Contraindicated with melphalan due to severe blood-containing diarrhea
- Probenecid interaction may elevate serum concentration
- Potential potentiation with Chlorpromazine and Perphenazine
Contraindications
- Contradicted in infants under 3 months
- Contraindicated in epilepsy and CNS lesions
Side Effects
- Gastro-intestinal disturbances
- Haemolysis in G6PD deficiency
- Allergic reactions including urticaria and rashes
- Muscle weakness, phototoxicity, jaundice, visual disturbances
- Convulsions
Pregnancy & Lactation
- Potential for causing cartilage and DNA damage
- Excretion in breast milk
- Reported hemolytic anemia in breastfed child of an azotemic mother
Precautions & Warnings
- Consider risk-benefit during first trimester and breastfeeding
- Consider in case of impaired renal or hepatic function
