Indications
- Treatment of urinary tract infection caused by susceptible Gram-negative organisms
- Effective against Proteus species, Klebsiella species, Enterobacter species, and Escherichia coli
Pharmacology
- Synthetic narrow spectrum antibacterial
- Bacteriostatic or bactericidal depending on concentration
- Inhibits bacterial DNA synthesis
- Rapid and complete absorption from the G.I. Tract
- Distributed to most tissues, especially to the kidney and urine
- Half-life of 1.1 to 2.5 hours during normal renal function
- Half-life of up to 21 hours in impaired renal function
- Rapid and almost complete excretion within 24 hours
Dosage & Administration
- Adult dosage: 1g every 6 hours initially, reducing to 500mg every 6 hours for 7 days
- Children dosage: Oral 13.75 mg per kg body weight every six hours for one or two weeks, maintenance: Oral 8.25 mg per kg body weight every six hours
Interaction
- Concomitant use with melphalan may cause severe blood-containing diarrhoea
- Probenecid inhibits tubular secretion and may elevate serum concentration
- Chlorpromazine and Perphenazine can potentiate the effect of Naligram
Contraindications
- Infants under 3 months
- Epilepsy
- CNS lesions
Side Effects
- Gastro-intestinal disturbances including nausea, vomiting, and diarrhoea
- Haemolysis in G6PD deficiency
- Allergic reactions including urticaria, rashes, and fever
- Arthralgia, eosinophilia
- Myalgia, muscle weakness
- Phototoxicity, jaundice
- Visual disturbances and convulsions
Pregnancy & Lactation
- Possibility of causing cartilage damage
- Possibility of causing DNA damage due to its DNA-gyrase inhibiting properties
- Excretion in breast milk
- Report of hemolytic anaemia in a breastfed child of an azotemic mother
Precautions & Warnings
- Consider risk-benefit during the first trimester of pregnancy and during breastfeeding
- Imaired renal or hepatic function
