Indications

Urinary tract infection caused by susceptible Gram-negative organisms

Pharmacology

Nalidixic acid is a synthetic narrow spectrum antibacterial
Inhibits bacterial DNA synthesis
Rapidly and completely absorbed from the G.I. Tract
Distributed to most tissues specially to the kidney and to the urine

Dosage & Administration

Adult: initial dose is 1 g every 6 hours for 7 days, reducing to 500 mg every 6 hours
Children: Initial oral dosage for infants and children 3 months of age and over is 13.75 mg per kg body weight every six hours for one or two weeks

Interaction

Concomitant use with melphalan may cause severe blood-containing diarrhea caused by hemorrhagic ulcerative colitis
Probenecids inhibits tubular secretion and may elevate serum concentration
Chlorpromazine and Perphenazine have been shown to potentiate the effect in vitro

Contraindications

Infants under 3 months
Epilepsy
CNS lesions

Side Effects

Gastro-intestinal disturbances including nausea, vomiting, diarrhea
Haemolysis in G6PD deficiency
Allergic reactions including urticaria, rashes, fever, arthralgia
Myalgia, muscle weakness, phototoxicity, jaundice, visual disturbances, convulsions

Pregnancy & Lactation

Possibility of cartilage damage and DNA damage
Excreted in breast milk
Report of hemolytic anemia in a breastfed child of an azotemic mother

Precautions & Warnings

Risk-benefit must be considered during the first trimester of pregnancy and during breastfeeding
Impaired renal or hepatic function
