Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in Chemoreceptor Trigger Zone and stomach
- Gastroprokinetic action
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates lower esophageal sphincter pressure
Dosage & Administration
- Should be taken 15-30 minutes before meals
- Adults dose: 10-20 mg every 6-8 hours daily, maximum 80 mg daily
- Children dose: 2-4 ml suspension/10 kg body weight every 6-8 hours daily
- Acute nausea and vomiting adult dose: 20 mg every 6-8 hours daily
- By rectum in suppositories for adults and children
Interaction
- Anticholinergic drugs may antagonize the antidyspeptic effect
- Avoid simultaneous administration with antacids and antisecretory drugs
- Metabolized through CYP3A4 pathway
- Concomitant use of CYP3A4 inhibitors may increase plasma levels
- Possible influence on absorption of concomitantly orally administered drugs
- Does not potentiate the action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Increase in plasma prolactin level
- Rare allergic reactions
- Possible neuroendocrinological phenomena
Pregnancy & Lactation
- Caution during first trimester of pregnancy
- Potential excretion in breast milk
- Lower concentrations in breast milk compared to plasma
Precautions & Warnings
- Absolute caution in the case of children
- Increased risk of extrapyramidal reactions in young children
- Caution in patients with hepatic impairment
Use in Special Populations
- Caution in infants due to underdeveloped blood-brain barrier
- Caution in patients with hepatic impairment
- Effect on dosing frequency in patients with severe renal insufficiency
Overdose Effects
- Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
- Administration of activated charcoal recommended
- Anticholinergic or antiparkinson drugs may be helpful in controlling the reactions
