Indications
- Treatment of functional symptoms of Benign Prostatic Hyperplasia (BPH)
Pharmacology
- Selective alpha1 adrenoceptor blocking agent
- Relaxes smooth muscle in the bladder neck and prostate
- Improves urine flow rate
- Reduces symptoms of BPH
- Complete absorption (90%) following oral administration under fasting conditions
- Time to maximum concentration (Tmax) reached by four to five hours under fasting conditions
- Extensively metabolized by cytochrome P 450 enzymes in the liver
- Elimination half-life of Tamsulosin hydrochloride ranges from five to seven hours
- Apparent half-life of Tamsulosin hydrochloride is approximately 9 to 15 hours
Dosage & Administration
- Tamsulosin Hydrochloride 0.4mg (one capsule) daily
- May be increased to Tamsulosin Hydrochloride 0.8mg (two capsules) once daily if necessary
- To be taken after meal at night
- Capsule should be swallowed whole with a glass of water (about 150ml) in standing or sitting position
Interaction
- Concurrent administration of other alfa1-adrenoceptor antagonists could lead to hypotensive effects
- No interactions seen with atenolol, enalapril, or nifedipine
- Cimetidine may increase plasma levels
- Frusemide may decrease plasma levels
- No interactions at the level of hepatic metabolism during in vitro studies with certain drugs
- Diclofenac and warfarin may increase the elimination rate
Contraindications
- Hypersensitivity to Tamsulosin hydrochloride
- History of orthostatic hypotension
- Severe hepatic insufficiency
- Potential reduction in blood pressure and risk of syncope
- Sit or lie down at first signs of orthostatic hypotension
- Examination to exclude other conditions before therapy initiation
- Regular examinations including digital rectal examination and Prostate Specific Antigen (PSA) determination
Side Effects
- Dizziness
- Abnormal ejaculation
- Less frequently: headache, asthenia, postural hypotension, palpitations
Pregnancy & Lactation
- Not recommended
Precautions & Warnings
- Transient postural symptoms rarely occurred during orthostatic provocation testing after the first dose
- Use not advised in patients with micturition syncope
- Effect on ability to drive and use machines is not fully known
Overdose Effects
- Acute hypotension likely to occur after overdosage
- Cardiovascular support should be given
- Volume expanders and vasopressors could be employed if necessary
- Renal function should be monitored
- General supportive measures should be applied
- Measures to impede absorption and gastric lavage can be considered
- Activated charcoal and osmotic laxative can be administered
