Indications
- Treatment of functional symptoms of Benign Prostatic Hyperplasia (BPH)
Pharmacology
- Selective alpha1 adrenoceptor blocking agent
- Relaxes smooth muscles in bladder neck and prostate
- Improvement in urine flow rate
- Reduction in symptoms of BPH
- Complete absorption (90%) under fasting conditions
- Time to maximum concentration reached by four to five hours under fasting conditions
- Extensively metabolized by cytochrome P 450 enzymes in the liver
- Elimination half-life ranges from five to seven hours
Dosage & Administration
- Tamsulosin Hydrochloride 0.4 mg (one capsule) daily
- To be taken after meal at night
- Dose may be increased to 0.8 mg (two capsules) once daily if necessary
- Swallowed whole with a glass of water in standing or sitting position
- Not to be crunched or chewed
Interaction
- Concurrent administration with other alfa1-adrenoceptor antagonists could lead to hypotensive effects
- No interactions seen with atenolol, enalapril, or nifedipine
- Cimetidine brings about a rise, frusemide brings about a fall in plasma levels of Urolosin
- No interactions at the level of hepatic metabolism seen during in vitro studies with liver microsomal fractions
- Diclofenac and warfarin may increase the elimination rate of Urolosin
Contraindications
- Hypersensitivity to Tamsulosin hydrochloride
- History of orthostatic hypotension
- Severe hepatic insufficiency
- Reduction in blood pressure can occur in individual cases
- Avoid situations where injury could result
- Patient should be examined to exclude other conditions which can cause the same symptoms as Benign Prostatic hyperplasia
- Digital rectal examination and determination of Prostate Specific Antigen (PSA) should be performed before treatment and at regular intervals afterwards

- Headache
- Asthenia
- Postural hypotension
- Palpitations
Pregnancy & Lactation
- Not recommended for use during pregnancy and lactation
Precautions & Warnings
- Transient postural symptoms have occurred during orthostatic provocation testing after the first dose
- Use in patients with micturition syncope is not advised
- No data available on whether Urolosin adversely affects the ability to drive or operate machines
- Patients should be aware of the possibility of dizziness
Overdose Effects
- Acute hypotension likely to occur
- Cardiovascular support should be given
- Blood pressure can be restored and heart rate brought back to normal by lying the patient down
- Volume expanders and vasopressors could be employed if necessary
- Renal function should be monitored
- General supportive measures should be applied
- Dialysis unlikely to be of help as Urolosin is very highly bound to plasma proteins
- Measures such as emesis, gastric lavage, activated charcoal, and osmotic laxative can be taken to impede absorption
