Indications
- Prophylaxis of gastric and duodenal ulceration in NSAID users at high risk
- Healing of established NSAID-induced gastric and duodenal damage
- Induction of labor
- Prevention and treatment of postpartum hemorrhage
Pharmacology
- Misoprostol is extensively absorbed
- Rapid de-esterification to its free acid
- Maximum plasma concentrations of Misoprostol acid are diminished when taken with food
- NSAIDs inhibit prostaglandin synthesis
- Misoprostol can increase bicarbonate and mucus production
Dosage & Administration
- Benign gastric and duodenal ulceration and NSAID associated ulceration: 800 mcg daily
- Prophylaxis of NSAID-induced gastric and duodenal ulcer: 200 mcg 2-4 times daily
- Induction of labor: 25 mcg in the posterior fornix of the vagina
- Prevention of postpartum hemorrhage: 600 mcg orally immediately following delivery
Interaction
- No evidence of clinically significant interaction with cardiac, pulmonary, CNS drugs and NSAIDs
- Bioavailability is decreased with high doses of antacid
Contraindications
- Contraindicated in anyone with a history of allergy to prostaglandins
- Contraindicated in pregnancy
Side Effects
- Frequent adverse effects involve the GI tract such as diarrhea, abdominal pain, dyspepsia, flatulence, nausea, vomiting, rashes and dizziness
Pregnancy & Lactation
- Contraindicated in pregnant women
- Prescribed with precautions and warnings for women of childbearing potential
Precautions & Warnings
- Cytomis is contraindicated in pregnant women
- Induction of labor has specific contraindications related to fetal distress and placenta issues
Use in Special Populations
- Safety and effectiveness in children below the age of 18 years have not been established
Overdose Effects
- Toxic dose in human has not been determined
- Clinical signs of overdose include sedation, tremor, convulsions, dyspnea, and diarrhea
