Indications

  • Prophylaxis of gastric and duodenal ulceration in NSAID users
  • Healing of established NSAID-induced damage
  • Induction of labor
  • Prevention and treatment of postpartum hemorrhage

Pharmacology

  • Extensively absorbed
  • Undergoes rapid de-esterification
  • Antisecretory properties
  • Mucosal protective properties
  • Increases bicarbonate and mucus production
  • Diminished plasma concentrations with food intake
  • Total availability reduced by use of concomitant antacid

Dosage & Administration

  • 800 mcg daily for benign gastric and duodenal ulceration
  • 200 mcg 2-4 times daily for prophylaxis of NSAID-induced ulcer
  • 25 mcg in the posterior fornix or 100 mcg orally for labor induction
  • 600 mcg orally for prevention of postpartum hemorrhage
  • 600-1000 mcg orally or rectally for treatment of postpartum hemorrhage

Interaction

  • No clinically significant interaction with cardiac, pulmonary, CNS drugs or NSAIDs
  • Bioavailability decreased with high doses of antacid

Contraindications

  • Allergy to prostaglandins
  • Pregnancy

Pregnancy & Lactation

  • Contraindicated in pregnant women

  • Contraindicated in pregnant women for NSAID therapy
  • Effective contraception method required for women of childbearing potential
  • Induction of labor criteria: 38 weeks gestation, fetal distress contraindicated
  • Vertex presentation required for fetus

Use in Special Populations

  • Not established for children below 18 years old

Overdose Effects

  • Toxic dose not determined
  • Clinical signs of overdose include sedation, tremor, convulsions, dyspnea, abdominal pain, diarrhea, fever
  • Symptoms should be treated with supportive therapy
