Description
- Misoprostol is indicated for prophylaxis of gastric and duodenal ulceration in NSAID users at high risk of complications, healing of established NSAID-induced gastric and duodenal damage, induction of labor, prevention and treatment of postpartum hemorrhage.
Dosage and Administration
- 800 mcg daily for benign gastric and duodenal ulceration, 200 mcg 2-4 times daily for prophylaxis of NSAID-induced ulcer, 25 mcg in the posterior fornix of the vagina for labor induction, 600 mcg orally for prevention of postpartum hemorrhage, 600 mcg orally or 1000 mcg per rectally for treatment of postpartum hemorrhage.
Pharmacology
- Misoprostol is extensively absorbed and undergoes rapid de-esterification to its free acid, which is responsible for its clinical activity. It has both antisecretory and mucosal protective properties.
Side Effects
- The most frequent adverse effects are associated with the GI tract such as diarrhea, abdominal pain, dyspepsia, flatulence, nausea, vomiting, rashes, and dizziness.
Contraindications
- Contraindicated in individuals with a history of allergy to prostaglandins and during pregnancy.
Pregnancy and Lactation
- Contraindicated in pregnant women due to its abortifacient property. Excretion of the active metabolite into milk is possible but has not been studied.
Interaction
- No evidence of clinically significant interaction with cardiac, pulmonary, CNS drugs, and NSAIDs. Bioavailability is decreased with high doses of antacid.
Precautions and Warnings
- Contraindicated in pregnant women for prevention and treatment of NSAID-induced ulcer. Effective contraception must be used by women of childbearing potential during therapy.
Precaution for Induction of Labor
- G-G-Misoprostol should be used with caution and only after specific conditions have been met for induction of labor.
Use in Children
- Safety and effectiveness not established in children below 18 years.
Overdose Effects
- Toxic dose not determined. Clinical signs that may indicate an overdose are sedation, tremor, convulsions, dyspnea, abdominal pain, diarrhea, and fever.
