Indications
- Prophylaxis of gastric and duodenal ulceration in NSAID users
- Healing of established NSAID-induced gastric and duodenal damage
- Induction of labor
- Prevention and treatment of postpartum hemorrhage
Pharmacology
- Rapid de-esterification to its free acid
- Maximum plasma concentrations reduced when taken with food
- Both antisecretory and mucosal protective properties
- Increases bicarbonate and mucus production
Dosage & Administration
- 800 mcg daily for benign gastric and duodenal ulceration
- 200 mcg 2-4 times daily for prophylaxis of NSAID-induced ulcer
- 25 mcg in the posterior fornix of the vagina for labor induction
- 600 mcg orally for prevention of postpartum hemorrhage
Interaction
- No evidence of clinically significant interaction with certain drugs
- Bioavailability decreased with high doses of antacid
Contraindications
- History of allergy to prostaglandins
- Pregnancy
Side Effects
- Adverse effects involve the GI tract
- Incidence of diarrhea may be minimized by administering the drug after meals
Pregnancy & Lactation
- Contradicted in pregnant women
- Prescribed with strict preconditions for women of childbearing potential
- Excretion of the active metabolite into milk possible but not studied
Precautions & Warnings
- Contraindicated in pregnant women
- Effective contraception must be used by women of childbearing potential
