Indications

Description
- Synthetically prepared Pitocin DS
- Identical to natural occurring hormone from the posterior pituitary gland
- Causes contractions of the uterus
- Mimics contractions of normal labor
- Impedes uterine blood flow
- Stimulates lactating breast
- Minimal cardiovascular and antidiuretic properties
Pharmacology
- Promotes contractions by increasing intracellular Ca2+
- Specific receptors in muscle lining of uterus
- Receptor concentration increases during pregnancy
Dosage & Administration
- Intravenous infusion under adequate control
- 1-5 IU added to 500 ml of infusion fluid
- Started at 1 mU per minute
- Dosage can be doubled every 30-45 minutes
- Intramuscular administration may lead to irregular response
- Dosage should not exceed 0.5-1 IU per injection
Interaction
- Severe hypertension reported when given with vasoconstrictor
- Cyclopropane anesthesia may modify cardiovascular effects
- Maternal sinus bradycardia with abnormal rhythms
Contraindications
- Cephalo-pelvic disproportion
- Abnormal position of the child or skull
- Overdistention of the uterus
- Severe toxaemia
- Hypertonic uterine dysfunction
- Foetal distress
- Cord presentation and prolapse
- Placenta previa
- Vasa praevia
Side Effects
- Uterine hypertonicity
- Spasm
- Rupture of the uterus
- Increased blood loss
- Afbrinigenemia
- Severe water intoxication
- Nausea
- Vomiting
- Haemorrhage
- Cardiac arrhythmias
- Anaphylactic reaction
- Increased incidence of icterus neonatorum
Pregnancy & Lactation
- Pregnancy category C
- Unknown excretion in human milk
Precautions & Warnings
- Uterine sensitivity may vary widely
- Smallest effective dose must be determined for each patient
- Should only be used with adequate medical supervision
- Care with scars of uterus and cervix
Overdose Effects
- Violent uterine contractions
- Fetal bradycardia
- Arrhythmias
- Uterine rupture
- Fetal or maternal death
- Immediate discontinuation of infusion
- Induction of tocolysis
Therapeutic Class

Reconstitution
- Potency maintained for at least 24 hours when diluted with specific infusion fluids
- No precipitation of ingredients in specific infusion fluids
- 24-hours stability not established in certain fluids
