Indications

Description
Syntocin contains synthetically prepared oxytocin, which mimics contractions of normal labor and stimulates lactation without affecting milk secretion. It has minimal cardiovascular and antidiuretic properties.
Pharmacology
Syntocin promotes uterine contractions by increasing intracellular Ca2+ and activating myosin's light chain kinase through oxytocin receptors in the uterine muscle lining.
Dosage & Administration
- Induction of labor, stimulation of uterine contractions: Intravenous infusion starting at 1 mU per minute, increasing dosage every 30-45 minutes as needed.
- Post delivery period, for prevention or treatment of secondary haemorrhage and adherent placenta: 2-5 IU by intramuscular injection or intravenous infusion.
- To stimulate lactation: 2 IU by intramuscular injection, only if advised by a registered Gynaecologist/Physician.
- Treatment of incomplete abortion: 2-5 IU every 30-60 minutes by intramuscular injection or intravenous infusion.
Interaction
Severe hypertension has been reported when Syntocin was given following prophylactic administration of a vasoconstrictor in conjunction with caudal-block anesthesia. Cyclopropane anesthesia may modify Syntocin’s cardiovascular effects, potentially causing hypotension.
Contraindications
- Cephalo-pelvic disproportion
- Abnormal fetal position or skull position
- Overdistention of the uterus
- Severe toxaemia
- Hypertonic uterine dysfunction
- Fetal distress without imminent delivery
- Cord presentation and prolapse
- Total placenta previa
- Vasa praevia
Side Effects
- Hypersensitivity resulting in uterine hypertonicity or rupture
- Increased blood loss
- Severe water intoxication
- Nausea, vomiting, haemorrhage, cardiac arrhythmias
- Anaphylactic reaction
- Increased incidence of icterus neonatorum
Pregnancy & Lactation
Pregnancy category C. Excretion in human milk is unknown.
Precautions & Warnings
- Careful use due to wide uterine sensitivity
- Adequate medical supervision required
- Monitoring of fetal heart rate and maternal blood pressure
- Caution if uterine or cervical scars present
Overdose Effects
Excess Syntocin may cause violent uterine contractions leading to fetal bradycardia and arrhythmias, extensive laceration, uterine rupture, and potential fetal or maternal death. Tocolysis should be induced immediately.

Reconstitution
Syntocin retains potency for at least 24 hours when diluted with compatible infusion fluids such as sodium chloride 0.9% or anhydrous glucose 5%. It can be injected directly into the infusion line without causing precipitation in these fluids. Stability in other infusion fluids has not been fully established.
