Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
- Gastroprokinetic action
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates lower esophageal sphincter pressure
Dosage & Administration
- Should be taken 15-30 minutes before meals
- Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily
- Maximum recommended daily dose for adults: 80 mg
- Dosing for children based on body weight
- Administration by rectum in suppositories
- Maximum period of treatment is 12 weeks
Interaction
- Antagonizes antidyspeptic effect of anticholinergic drugs
- Lower oral bioavailability with simultaneous antacids and antisecretory drugs administration
- Metabolic pathway through CYP3A4
- Concomitant use of CYP3A4 inhibitors may increase plasma levels of domperidone
- Potential influence on absorption of concomitantly orally administered drugs
- Does not potentiate the action of neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to domperidone
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction
- Perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Increase in plasma prolactin level
- Rare allergic reactions like rash and urticaria
Pregnancy & Lactation
- Use during first trimester justified by therapeutic benefit
- No evidence of increased risk of malformations in humans
- Excretion in breast milk
- Not recommended for nursing mothers unless benefits outweigh potential risk
Precautions & Warnings
- Caution in the case of children due to risk of extra-pyramidal reactions
- Caution in patients with hepatic impairment
Use in Special Populations
- Caution in infants due to incomplete development of metabolic and blood-brain barrier functions
- Caution in liver disorders
- Caution in severe renal insufficiency
Overdose Effects
- Symptoms: drowsiness, disorientation, extrapyramidal reactions
- Administration of activated charcoal recommended
- Anticholinergic drugs or antihistamines may be helpful in controlling extrapyramidal reactions
