Pharmacology

  • Dexamethasone has anti-inflammatory and anti-allergic action. Chloramphenicol is a broad spectrum bacteriostatic antibiotic.

Dosage and administration

  • For bacterial conjunctivitis: 1-2 drops every 2 hours for 2 days, then 1-2 drops every 4 hours for 5 days. For corneal ulcers: 2 drops every 15 minutes for 6 hours, then every 30 minutes for the first day, followed by hourly instillation on the second day, and then every 4 hours for 3-14 days. For ear infections: 2-3 drops every 2-3 hours, adjusted based on clinical signs improvement.

Interactions

  • Chloramphenicol may interact with anticoagulants like warfarin. Dexamethasone's efficacy may be reduced with phenytoin, phenobarbitone, ephedrine, and rifampicin.

Contraindications

  • Epithelial herpes simplex cornealis, fungal, viral, tuberculous, and other eye infections, glaucoma, myelosuppression during previous exposure to Chloramphenicol, hypersensitivity to Chloramphenicol and Dexamethasone Phosphate.

Side effects

  • Transient ocular burning or discomfort, stinging, redness, itching, conjunctivitis, foreign body sensation, photophobia, blurred vision, dryness, eye pain, possible elevation of intraocular pressure, optic nerve damage, and posterior subcapsular cataract formation.

Pregnancy and lactation

  • Caution needed during pregnancy and lactation. No adequate and well-controlled studies in pregnant women.

Precautions and warnings

  • Consider persistent fungal infections after prolonged corticosteroid dosing, monitor intraocular pressure during prolonged use, watch for overgrowth of non-susceptible organisms, and discontinue if new infections appear.

Storage conditions

  • Keep in a cool and dry place, away from light, and out of the reach of children. Do not use after 30 days of first opening.
