Pharmacology
- Dopamine antagonist
- Block dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
- Gastroprokinetic action
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates lower esophageal sphincter pressure
Dosage & Administration
- Should be taken 15-30 minutes before meals
- Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily
- Maximum dose for adults: 80 mg daily
- Usual recommended oral dose for children
- Maximum period of treatment for acute nausea and vomiting: 12 weeks
- Rectal administration for adults and children
- Maximum daily dose rectally in children: 30 mg
Interaction
- Antagonizes antidyspeptic effect of domperidone with anticholinergic drugs
- Lower oral bioavailability with antacids and antisecretory drugs
- Main metabolic pathway through CYP3A4
- Concomitant use of CYP3A4 inhibitors may increase plasma levels of domperidone
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction
- Perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Increase in plasma prolactin level
- Hyperprolactinaemia may give rise to neuroendocrinological phenomena
- Rare allergic reactions (rash, urticaria)
Pregnancy & Lactation
- Should only be used during the first trimester of pregnancy if justified by therapeutic benefit
- No evidence of increase in risk of malformations in humans
- Excreted in breast milk of lactating rats
- Domperidone concentrations in breast milk are 4 times lower than corresponding plasma concentrations
- Nursing not recommended unless expected benefits outweigh potential risk
- Does not affect mental alertness
Precautions & Warnings
- Increased risk of extra-pyramidal reactions in young children
- Caution in patients with hepatic impairment
Use in Special Populations
- Caution in infants due to incomplete blood-brain barrier development
- Caution in liver disorders
- Caution in severe renal insufficiency
Overdose Effects
- Symptoms may include drowsiness, disorientation, extrapyramidal reactions
- Administer activated charcoal and observe the patients
