Indications

  • Acute purulent conjunctivitis
  • Corneal ulceration
  • Keratitis disciformis
  • Post-herpetic keratitis
  • Allergic conjunctivitis
  • Otitis externa
  • Otitis media
  • Chronic suppurative otitis media

Pharmacology

  • Anti-inflammatory action
  • Anti-allergic action
  • Glucocorticoid
  • Broad spectrum bacteriostatic antibiotic
  • Inhibition of bacterial protein synthesis

Dosage & Administration

  • Eye:
    • Bacterial Conjunctivitis: 1-2 drops every 2 hours for 2 days, then 1-2 drops every 4 hours for the next 5 days
    • Corneal Ulcers: 2 drops every 15 minutes for first 6 hours, then 2 drops every 30 minutes for the rest of the first day, followed by different regimen for days 2-14
  • Ear: 2-3 drops every 2-3 hours initially, with decreased frequency as warranted by improvement

Interaction

  • Increase in anticoagulant effect when absorbed from eye
  • Inhibition of action with Phenobarbital
  • Reduced efficacy with concurrent administration of phenytoin, phenobarbitone, ephedrine, and rifampicin

Contraindications

  • Epithelial herpes simplex cornealis
  • Fungal, viral, tuberculous and other eye infections
  • Glaucoma
  • Myelosuppression during previous Chloramphenicol exposure
  • Hypersensitivity to Chloramphenicol & Dexamethasone Phosphate

  • Eye pain
  • Elevation of intraocular pressure
  • Possible development of glaucoma

Caution with use during pregnancy and nursing

Precautions & Warnings

  • Consideration of persistent fungal infections of the cornea
  • Monitoring of intraocular pressure
  • Potential for posterior subcapsular cataract formation and glaucoma with prolonged steroid use
  • Overgrowth of non-susceptible organisms with prolonged antibiotic use
  • Need for discontinuation and alternative therapy in case of new infections
