Indications

Pharmacology

  • Dexamethasone as glucocorticoid
  • Anti-inflammatory action of Dexamethasone
  • Prostaglandin synthesis reduction
  • Inhibition of neutrophil infiltration
  • Bactericidal action of Ciprofloxacin
  • Activity against gram-negative and gram-positive organisms
  • Interference with DNA gyrase

Dosage & Administration

  • Eye: Instill 1 drop every 4-6 hours
  • Increased initial dosage for first 24-48 hours
  • Ear: 4 drops instilled 2 times daily for 7 days
  • Usage in pediatric patients
  • Safety and effectiveness in pediatric patients below 1 year not established

Interaction

  • Elevation of plasma concentrations of theophylline
  • Interference with caffeine metabolism
  • Enhancement of warfarin effects
  • Transient elevations in serum creatinine with cyclosporin concomitantly

Contraindications

  • Hypersensitivity to any ingredient
  • Herpes simplex and other viral conditions
  • Mycosis
  • Glaucoma
  • Newborn babies
  • Fungal diseases of ocular or auricular structures

Side Effects

  • Transient ocular burning or discomfort
  • Ocular stinging, redness, itching, conjunctivitis
  • Elevation of intraocular pressure
  • Development of glaucoma
  • Optic nerve damage
  • Posterior subcapsular cataract formation
  • Delayed wound healing
  • Ear discomfort and pain
  • Irritability, dizziness, erythema

Pregnancy & Lactation

  • Use during pregnancy if potential benefit justifies risk
  • Caution with nursing mothers due to unknown excretion in human milk

Precautions & Warnings

  • Possibility of fungal infections after long-term steroid dosing
  • Prolonged use may result in overgrowth of non-susceptible organisms
  • Monitoring of intraocular pressure required
  • Caution for prolonged use of ophthalmic steroids
  • Potential for glaucoma, optic nerve damage, cataract formation
