Description
- A dopamine antagonist medication used to treat nausea and vomiting.

Dose Administration
- Should be taken 15-30 minutes before meals and, if necessary, before retiring.
Recommended Oral Dose
- Adults: 10-20 mg (1-2 tablet or 10-20 ml suspension), every 6-8 hours daily. The maximum dose of Domperidone is 80 mg daily.
- Children: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml pediatric drops/10 kg body weight, every 6-8 hours daily.
Contraindications
- Hypersensitivity to the drug, neonates, gastrointestinal hemorrhage, mechanical obstruction, perforation, and prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Extrapyramidal phenomena
- Increase in plasma prolactin level
- Rash and urticaria
Mode of Action
- Dopamine antagonist
- Blocks dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying
- Increases esophageal peristalsis and lower esophageal sphincter pressure
Interaction
- Antagonizes antidyspeptic effect of anticholinergic drugs
- Lower oral bioavailability when given simultaneously with antacids and antisecretory drugs
- Main metabolic pathway is through CYP3A4
- In vitro data suggest concomitant use of CYP3A4 inhibitors may result in increased plasma levels
Pregnancy & Lactation
- Should only be used during the first trimester of pregnancy if justified by therapeutic benefit
- Excreted in breast milk of lactating rats
- Domperidone concentrations in breast milk 4 times lower than corresponding plasma concentrations
Precautions & Warnings
- Use with absolute caution in children due to the risk of extra-pyramidal reactions
- Use with caution in patients with hepatic impairment
