Indications
- Dilates pupil for eye examinations
- Used in cycloplegic refraction
- Used in examination of the fundus of the eye
- Used before and after eye surgery
Pharmacology
- Induces mydriasis and cycloplegia
- Paralyzes the parasympathetic system
- Acts by blocking the action of the parasympathetic nervous system
- Competes with acetylcholine for uptake at receptor sites
Dosage & Administration
- Instill 1 or 2 drops for refraction, repeated in 5 minutes
- Additional drop may be instilled to prolong mydriatic effect if patient not seen within 20-30 minutes
- Instill 1 drop 15-20 minutes prior to examination of the fundus
Interaction
- Atropine like substances
- Tricyclic antidepressants
- Numerous H1 antihistaminic substances
- Anti-cholinergic agents
- Anti-Parkinson drugs
- Phenothiazines
- Neuroleptic drugs capable of reinforcing the effect of Mydrimide
Contraindications
- Narrow angle glaucoma
- Hypersensitivity to any component of the product
Side Effects
- Slight burning sensation after instillation into the eye
- Psychotic episodes and behavioral changes in children
- Cardio-respiratory collapse in children
- Impairs performance and responsiveness while driving or operating machinery
- Increase in intraocular pressure
- Allergic reaction
- Dryness of the mouth
- Tachycardia
- Headache
Pregnancy & Lactation
- Pregnancy Category C
- Administer to pregnant woman only if clearly needed
- Caution in nursing women as excretion in human milk is not known
Precautions & Warnings
- Consider administration of an antidote (miotic) after examination
- Patient should compress tear duct with finger for 2-3 minutes after instillation to reduce excessive systemic absorption
- Use with care in older patients
Overdose Effects
- Red face
- Accelerated pulse
- Dry mouth
- Reduced sweating and secretion
- Increase in body temperature
- Contact emergency doctor in case of accidental ingestion
- Administration of miotic agent (antidote) if required
