Description

  • Dopamine antagonist medication

  • Restores normal motility and tone of the upper gastrointestinal tract
  • Facilitates gastric emptying
  • Enhances antral and duodenal peristalsis
  • Regulates contraction of the pylorus
  • Increases esophageal peristalsis
  • Regulates lower esophageal sphincter pressure
  • Blocks dopamine receptors in the Chemoreceptor Trigger Zone (CTZ) and stomach
  • Gastroprokinetic action based on its blocking effect of dopamine receptors affecting gastrointestinal motility
  • Weak penetration across the blood-brain barrier, leading to minimal impact on dopaminergic receptors in the brain

Dosage and Administration

  • Should be taken 15-30 minutes before meals
  • Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily, with a maximum dose of 80 mg daily
  • Usual recommended oral dose for children: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml pediatric drops/10 kg body weight, every 6-8 hours daily
  • Specific dosage for dyspeptic symptom and acute/sub-acute conditions
  • Rectal administration for adults and children

Pharmacology

Interaction

  • Antagonized by anticholinergic drugs
  • Not to be given simultaneously with antacids and antisecretory drugs
  • Metabolism through CYP3A4, caution with drugs inhibiting this enzyme
  • Potential influence on absorption of concomitantly orally administered drugs, especially sustained release or enteric coated formulations
  • No potentiation of action of neuroleptics
  • Suppresses unwanted peripheral effects of dopaminergic agonists

Contraindications

  • Known hypersensitivity to the drug
  • Neonates
  • Gastrointestinal hemorrhage
  • Mechanical obstruction or perforation
  • Prolactin releasing pituitary tumor (prolactinoma)

Side Effects

  • Rare side effects include transient intestinal cramps and extrapyramidal phenomena
  • Possible increase in plasma prolactin level
  • Rare allergic reactions such as rash and urticaria

Pregnancy and Lactation

  • Usage during the first trimester of pregnancy may be justified if therapeutic benefit outweighs risk
  • Excreted in breast milk, but concentration is 4 times lower than corresponding plasma concentrations

Precautions and Warnings

  • Caution with use in children due to risk of extra-pyramidal reactions
  • Caution in patients with hepatic impairment

Use in Special Populations

  • Caution required in infants due to incompletely developed blood-brain barrier and immature/impaired blood-brain barrier
  • Use with caution in patients with hepatic impairment
  • Use with caution in patients with severe renal insufficiency, adjustments in dosing frequency may be needed

Overdose Effects

  • Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
  • Recommended administration of activated charcoal and close patient observation
