Indications
- Treatment of premature ejaculation (PE) in men 18 to 64 years of age
- Persistent or recurrent ejaculation with minimal sexual stimulation
- Marked personal distress or interpersonal difficulty as a consequence of PE
- Poor control over ejaculation
Pharmacology
- Inhibition of neuronal reuptake of serotonin
- Potentiation of neurotransmitter's action
- Mechanism of action linked to sympathetic nervous system
- Rapid absorption with maximum plasma concentrations occurring 1-2 hours after intake
- Absolute bioavailability of 42%
- Extensive metabolization to multiple metabolites
- Rapid distribution with a mean steady state volume of distribution of 162 L
- Terminal half life of approximately 19 hours
Dosage & Administration
- Recommended starting dose for adult men (18 to 64 years of age) is 30 mg
- Maximum recommended dosing frequency is once every 24 hours
- Dose may be increased to the maximum recommended dose of 60 mg if the effect of 30 mg is insufficient
- Dapoxetine may be taken with or without food
- Not recommended for use in individuals below 18 years of age
- Caution advised in patients with mild or moderate renal impairment
- Contraindicated in patients with moderate and severe hepatic impairment
Interaction
- Caution advised if concomitant administration of Impreja and CNS active medicinal products is required
- Prescribed with caution in patients who use PDE5 inhibitors
- Prescribed with caution in patients who use alpha adrenergic receptor antagonists
- Caution advised when used in patients taking Warfarin chronically
- Patients advised to avoid alcohol while taking Impreja
Contraindications
- Known hypersensitivity to Dapoxetine Hydrochloride
- Significant pathological cardiac conditions
- Concomitant treatment with monoamine oxidase inhibitors (MAOIs), thioridazine, or serotonergic medicinal/herbal products
Side Effects
- Common effects include nausea, dizziness, dry mouth, headache, diarrhea, and insomnia
- Low rates of sexual dysfunction
- Very mild adverse effects on loss of libido and erectile dysfunction
Pregnancy & Lactation
- Not indicated for use by women
- Unknown excretion of dapoxetine or its metabolites through human breast milk
Precautions & Warnings
- Patients advised not to use Impreja in combination with recreational drugs
- Patients advised to avoid alcohol while taking Impreja
- Possibility of syncope and orthostatic hypotension
- Caution advised in patients with a history of bleeding or coagulation disorders
- Caution advised in concomitant use with PDE5 inhibitors and alpha adrenergic receptor antagonists
Overdose Effects
- No reported case of overdose
- Symptoms of overdose with SSRIs include serotonin-mediated adverse reactions
