Description

Selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)

Mechanism of Action

Increases levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood into the penile tissues

Pharmacokinetics

Rapidly absorbed after oral administration
Mean absolute bioavailability of 41%
Eliminated predominantly by hepatic metabolism
Maximum observed plasma concentrations reached within 30 to 120 minutes of oral dosing in the fasted state

Dosage & Administration

Recommended dose of 50 mg for erectile dysfunction
Maximum recommended dose of 100 mg or decreased to 25 mg
Recommended dose of 20 mg three times a day for pulmonary arterial hypertension

Metabolism & Clearance

Metabolized by cytochrome P450 isoenzymes
Excreted as metabolites predominantly in feces

Interaction

Metabolism mediated by CYP3A4 and CYP2C9
Inhibitors of these isoenzymes reduce clearance
Inducers of these isoenzymes increase clearance
