Indications

  • Bronchial asthma
  • Bronchospasm
  • Chronic obstructive pulmonary disease (COPD)
  • Pulmonary disease with spastic bronchial component

Pharmacology

  • Doxophylline selectively inhibits phosphodiesterase 4
  • Inhibition of platelet activating factor (PAF) and generation of leukotriene production
  • Different affinity towards adenosine A1 and A2 receptors

Dosage & Administration

  • Elderly: 200 mg tablet two or three times daily
  • Adults: 400 mg tablet two or three times daily
  • Children:
    • >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
    • 6-12 years of age: 6-9 mg/kg body weight two times daily
    • Doxophylline SR tablet to be taken once daily for 400 mg daily dose

Interaction

  • With Xanthine Preparations: Not recommended
  • With caffeine-containing beverages and food: Limit consumption
  • With sympathomimetic drugs: Caution
  • With erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, propanolol: May reduce hepatic clearance
  • With phenytoin, other anticonvulsants, cigarette smoking: May increase clearance

Contraindications

  • Known hypersensitivity to Doxophylline or other xanthine derivatives
  • Acute myocardial infarction
  • Hypotension
  • Lactation

  • Insomnia
  • Tachycardia
  • Tachypnea
  • Hyperglycemia
  • Albuminuria
  • Severe cardiac arrhythmias in case of overdose

Pregnancy & Lactation

  • Effects during Pregnancy: Not sufficient clinical evidence, evaluate risk-benefit ratio
  • Effects during Lactation: Contraindicated

  • Factors affecting hepatic clearance:
    • Age
    • Congestive cardiac decompensation
    • Chronic obstructive pulmonary disease
    • Severe liver disease
    • Concomitant infections
  • Caution in patients with:
    • Cardiac disease
    • Hypertension
    • Elderly
    • Severe hypoxemia
    • Hyperthyroidism
    • Chronic corpulmonale
    • Congestive heart failure
    • Liver disease
    • Peptic ulcer
    • Renal impairment

Overdose Effects

Severe cardiac arrhythmias and tonic-clonic seizure
