Indications
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary disease with spastic bronchial component
Pharmacology
- Novel bronchodilator
- Structurally differs from Theophylline
- Selective inhibition of phosphodiesterase 4
- Relaxes bronchial smooth muscle
- Decreased affinities toward adenosine A1 and A2 receptors
- Inhibition of platelet activating factor (PAF)
- Inhibition of leukotriene production
Dosage & Administration
- Elderly dosage: 200 mg tablet two or three times daily
- Adult dosage: 400 mg tablet two or three times daily
- Children >12 years of age dosage: 10 ml syrup or 200 mg tablet two or three times daily
- Children 6-12 years of age dosage: 6-9 mg/kg body weight two times daily
- Doxophylline SR tablet to be taken once daily if the required daily dose is 400 mg
Interaction
- Not to be administered with other xanthine preparations
- Limit consumption of caffeine-containing beverages and food
- Caution with ephedrine or other sympathomimetic drugs
- Concurrent administration with erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, and propranolol may reduce hepatic clearance
- Phenytoin, other anticonvulsants, and cigarette smoking may increase clearance
Contraindications
- Known hypersensitivity to the drug or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- Lactation

Pregnancy & Lactation
- Animal tests show no interference with pre and postnatal growth
- Use during pregnancy should be evaluated on a risk-benefit basis
- Contraindicated during lactation
- No effect on the ability to drive or use machines
Precautions & Warnings
- Factors that may reduce hepatic clearance and increase plasma levels
- Caution with cardiac disease, hypertension, elderly, severe hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, and peptic ulcer
- Monitoring of blood levels recommended
- No risk of addiction or dependence
Overdose Effects
- Severe cardiac arrhythmias
- Tonic-clonic seizure
- Discontinuation of treatment required
- Symptomatic treatment of cardiovascular collapse
