Description

Doxoma is used to treat bronchial asthma, bronchospasm, chronic obstructive pulmonary disease (COPD), and pulmonary disease with spastic bronchial component. It selectively inhibits phosphodiesterase 4, relaxes bronchial smooth muscle, and has a better safety profile compared to Theophylline.

Indications

  • Pulmonary disease with spastic bronchial component

Pharmacology

  • Inhibits phosphodiesterase 4
  • Relaxes bronchial smooth muscle
  • Selective inhibition of adenosine A1 and A2 receptors
  • Inhibits platelet activating factor (PAF)
  • Reduces leukotriene production

Dosage and administration

  • Elderly:
    • 200 mg tablet two or three times daily
  • Adults:
    • 400 mg tablet two or three times daily
  • Children:
    • >12 years: 10 ml syrup or 200 mg tablet two or three times daily
    • 6-12 years: 6-9 mg/kg body weight two times daily

Interactions

  • Contraindicated with other xanthine preparations
  • Caution with ephedrine or other sympathomimetic drugs
  • May interact with erythromycin, TAO, lincomycin, clindamycin, allopurinol, cimetidine, influenza vaccine, propanolol, phenytoin, and other anticonvulsants

  • Known hypersensitivity to the drug or other xanthine derivatives
  • Acute myocardial infarction
  • Hypotension
  • Lactation

  • Caution in patients with cardiac disease, hypertension, elderly, severe hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, peptic ulcer, renal impairment

Overdose effects

  • Severe cardiac arrhythmias
  • Tonic-clonic seizure
  • Symptomatic treatment of cardiovascular collapse in case of overdose
