Description

Recombinant human erythropoietin (EPO), 165 amino acid sequence identical to human urinary EPO

Indications

  • Treatment of anemia in Chronic Kidney Disease (CKD) patients on dialysis and not on dialysis
  • Treatment of anemia in Zidovudine-treated HIV-infected patients
  • Reduction of allogeneic RBC transfusions in patients undergoing elective, noncardiac, nonvascular surgery
  • Prevention of anemia of prematurity in infants with specific birth weight and gestational age

Dosage

  • Iron Stores Evaluation: Evaluate iron status before and during treatment
  • CKD Patients: Monitor hemoglobin levels at least weekly until stable
  • Dose Adjustment: Adjust dose based on hemoglobin response
  • Administration Route:
    • Intravenously
    • Subcutaneously
  • Pediatric Dosing: Specific starting dose for pediatric patients

Administration

  • Preparation:
    • Do not shake
    • Protect syringe from light
    • Inspect visually for particulate matter before administration
  • Disposal: Discard unused portions of Erythropoietin alfa

Interaction

Simultaneous therapeutic administration with haematinic agents like ferrous sulphate may potentiate effect

Contraindications

  • Uncontrolled hypertension
  • Allergic reactions to Erythropoietin alfa
  • Pure Red Cell Aplasia (PRCA)

  • CKD Patients:
    • Hypertension
    • Arthralgia
    • Muscle spasm
    • Pyrexia
    • Dizziness
  • Zidovudine-treated HIV Patients:
    • Pyrexia
    • Cough
    • Rash
    • Injection site irritation
  • Cancer Patients on Chemotherapy:
    • Nausea
    • Vomiting
    • Myalgia
    • Arthralgia
    • Stomatitis
  • Surgery Patients:
    • Headache
    • Injection site pain
    • Chills
    • Deep vein thrombosis
  • Premature Infants: Fall in serum ferritin values

Pregnancy & Lactation

  • Pregnancy: Use only if potential benefit outweighs potential risk to the fetus
  • Lactation: Use with caution, considering benefit of breastfeeding and therapy to the woman

Overdose Effects

Can cause hemoglobin levels above desired level, leading to severe hypertension
