Pharmacological Action
- Dopamine antagonist
- Blocks dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Regulates lower esophageal sphincter pressure
- Prevents regurgitation of gastric content
Administration
- Should be taken 15-30 minutes before meals and, if necessary, before retiring
- Usual recommended oral dose for Adults: 10-20 mg every 6-8 hours daily
- Usual recommended oral dose for Children: 2-4 ml/10 kg body weight every 6-8 hours daily
- Rectal administration for Adults: 30-60 mg every 4-8 hours
- Rectal administration for Children: Up to 30 mg
Interactions
- Anticholinergic drugs may antagonize the antidyspeptic effect
- Simultaneous administration with antacids and antisecretory drugs lowers oral bioavailability
- Main metabolic pathway is through CYP3A4
- In vitro data suggests concomitant use of CYP3A4 inhibitors may increase plasma levels
- Influence on the absorption of orally administered drugs, particularly sustain release or enteric coated formulations
- Does not potentiate neuroleptics
- Suppresses unwanted peripheral effects of dopaminergic agonists
Contraindications
- Known hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage, mechanical obstruction, or perforation
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Increase in plasma prolactin level
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Should only be used during the first trimester if justified by therapeutic benefit
- No evidence of increase in risk of malformations
- Excreted in breast milk
- Not recommended for nursing mothers unless benefits outweigh potential risk
Precautions & Warnings
- Use with caution in children due to increased risk of extra-pyramidal reactions
- Use with caution in patients with hepatic impairment
Use in Special Populations
- Use in infants with great caution and under close medical supervision
- Use with caution in patients with liver and kidney disorders
Overdose Effects
- Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
- Administration of activated charcoal, and close observation recommended
- Anticholinergic, antiparkinson drugs or antihistamines may be helpful in controlling extrapyramidal reactions
