Dosage & Administration

  • Adult dose: 10-20 mg (1-2 tablets or 10-20 ml suspension) every 6-8 hours daily
  • Maximum adult dose: 80 mg daily
  • Children's dose: 2-4 ml suspension/10 kg body weight or 0.4-0.8 ml paediatric drops/10 kg body weight every 6-8 hours daily
  • Suppository dose for adults: 30-60 mg every 4-8 hours
  • Maximum suppository dose for children: 30 mg for those weighing 10 to 25 kg

Interaction

  • Concomitant administration of anticholinergic drugs may antagonize the antidyspeptic effect of domperidone
  • Antacids and antisecretory drugs should not be given simultaneously with domperidone
  • Metabolic pathway primarily through CYP3A4
  • Inhibition of CYP3A4 may increase plasma levels of domperidone
  • Potential influence on the absorption of concomitantly orally administered drugs
  • Examples of CYP3A4 inhibitors: azole antifungals, macrolide antibiotics, HIV protease inhibitors, nefazodone
  • Does not potentiate the action of neuroleptics
  • Suppresses unwanted peripheral effects of dopaminergic agonists

Side Effects

  • Rare occurrence of transient intestinal cramps
  • Rare occurrence of extrapyramidal phenomena
  • Potential increase in plasma prolactin level
  • Occasional allergic reactions like rash and urticaria

Pregnancy & Lactation

  • Use during the first trimester justified by the anticipated therapeutic benefit
  • No evidence of increase in the risk of malformations in humans
  • Excretion in breast milk, 4 times lower than corresponding plasma concentrations
  • Nursing not recommended unless expected benefits outweigh potential risk

Precautions & Warnings

  • Use with absolute caution in children
  • Increased risk of extrapyramidal reactions in young children
  • Highly metabolized in the liver
  • Use with caution in patients with hepatic impairment

Use in Special Populations

  • Use in infants with caution and under close medical supervision
  • Possible occurrence of neurological side effects in infants under 1 year of age
  • Use with caution in patients with liver disorders
  • Use with caution in patients with kidney disorders

Overdose Effects

  • Symptoms may include drowsiness, disorientation, and extrapyramidal reactions
  • Administration of activated charcoal and close observation recommended
  • Anticholinergic, antiparkinson drugs, or antihistamines may be helpful in controlling extrapyramidal reactions
