Description
- Dopamine antagonist medication for treating nausea and vomiting
- Gastroprokinetic action based on blocking effect of dopamine receptors
- Restores normal motility and tone of the upper gastrointestinal tract
- Facilitates gastric emptying and enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis and lower esophageal sphincter pressure
- Speeds barium transit in follow-through radiological studies
- Should be used with absolute caution in children
- Renal insufficiency impacts elimination half-life and plasma drug levels

Dosage & Administration
- To be taken 15-30 minutes before meals
- Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily
- Maximum dose for adults: 80 mg daily
- Recommended dose for children: 0.2-0.4 mg/kg body weight every 6-8 hours daily
- By rectum in suppositories: 30-60 mg every 4-8 hours for adults

Interaction
- Antagonized by concomitant administration of anticholinergic drugs
- Should not be given simultaneously with antacids and antisecretory drugs
- Main metabolic pathway through CYP3A4
- Could influence absorption of concomitantly orally administered drugs
- Does not potentiate the action of neuroleptics or dopaminergic agonists
Contraindications
- Hypersensitivity to the drug
- Neonates
- Gastrointestinal hemorrhage
- Mechanical obstruction or perforation
- Prolactin releasing pituitary tumor
Side Effects
- Transient intestinal cramps
- Rare extrapyramidal phenomena
- Increase in plasma prolactin level
- Hyperprolactinaemia may give rise to neuroendocrinological phenomena
- Rare allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Should only be used during the first trimester if justified by the therapeutic benefit
- No evidence of increase in the risk of malformations in humans
- Excreted in breast milk of lactating rats
- Not recommended for nursing mothers unless expected benefits outweigh potential risk
Precautions & Warnings
- Increased risk of extra-pyramidal reactions in young children
- Highly metabolized in the liver, caution required in hepatic impairment

- Great caution required for infants under 1 year of age due to incomplete blood-brain barrier development
