Hopetavir 400 mg (Tablet)

Indications

• Treatment of chronic hepatitis C infection
• Effective for HCV genotype 1, 2, 3, and 4
• Treatment of hepatocellular carcinoma meeting Milan criteria
• Treatment of HCV/HIV-1 co-infection

Pharmacology

• Nucleotide analog inhibitor
• Inhibits HCV NS5B RNA-dependent RNA polymerase
• Acts as a chain terminator in HCV viral replication
• Binds to Mg2+ ions in NS5B polymerase's GDD active site motif

Dosage & Administration

• One 400 mg tablet taken once daily
• Can be used in combination with Ribavirin or Pegylated Interferon
• Different treatment durations for different genotypes
• Special dosage considerations for patients with severe renal impairment

Interaction

• Reduced therapeutic effect with potent P-gp inducers
• Risk of serious symptomatic bradycardia when co-administered with amiodarone

Contraindications

• Contraindicated in pregnant women or men with pregnant partners
• Risks for birth defects and fetal death associated with combination therapies

Side Effects

• Common adverse events include fatigue, headache, nausea, insomnia, anemia, and rash

Pregnancy & Lactation

• Pregnancy Category B: No adequate and well-controlled studies
• Unknown presence of Sofosbuvir and its metabolites in human breast milk

Precautions & Warnings

• Risk of serious symptomatic bradycardia with amiodarone co-administration
• Cardiac monitoring is recommended in certain patient populations

Use in Special Populations

• Safety and effectiveness not established in children under 18 years of age
• Response rates in subjects over 65 similar to younger subjects

Overdose Effects

• No specific antidote available for overdose treatment
• General supportive measures including vital sign monitoring
