Indications

• improves glycemic control
• not recommended for diabetic ketoacidosis

Pharmacology

• regulation of glucose metabolism
• stimulates peripheral glucose uptake
• inhibits hepatic glucose production
• inhibits lipolysis and proteolysis
• enhances protein synthesis
• forms multihexamers when injected

Dosage & Administration

• ultra long-acting basal insulin
• once-daily at any time of the day
• requires individual dosage adjustments
• can be administered alone or in combination with other medications
• allows for flexibility in timing of administration

Interaction

• decreased hypoglycaemic effect with certain drugs
• increased hypoglycaemic effect with certain drugs
• decreased insulin resistance with certain drugs
• increased risk of weight gain and peripheral edema with certain drugs

Contraindications

• hypersensitivity to active substance or excipients
• hypoglycemia
• hyperglycemia
• eye disorder

Side Effects

• nasopharyngitis
• severe hypoglycemic episode
• upper respiratory tract infection
• headache
• diarrhea
• sinusitis
• injection site reactions
• peripheral edema

Pregnancy & Lactation

• potential risks to the fetus
• animal reproduction studies not always predictive of human response
• use in pregnant women should be justified by potential benefits to the mother
• unknown excretion in human milk
• insulin degludec secreted in rat milk

Precautions & Warnings

• must not be injected intravenously or intramuscularly
• no experience in children and adolescents under 18 years of age

Use in Special Populations

• no overall clinical differences in safety or effectiveness observed in elderly patients
• not indicated for use in pediatric population
