Indications
- Bronchial asthma
- Chronic bronchitis
- Emphysema
- Other lung diseases where bronchospasm is a complicating factor
Pharmacology
Bambuterol is a prodrug of terbutaline, a long-acting β2 adrenergic receptor agonist used in the treatment of asthma. It predominantly stimulates β2 receptors, producing relaxation of bronchial smooth muscle, inhibition of release of endogenous spasmogens, inhibition of edema caused by endogenous mediators, and increased mucociliary clearance.
Dosage & Administration
- Children 2-5 years: 10 mg (10 ml syrup)
- Children 6-12 years: 10 mg (10 ml syrup), may be increased to 20 mg
- Adult: Recommended initial dose 10 mg, may be increased to 20 mg after 1-2 weeks depending on clinical effect
Interaction
- Succinylcholin
- MAOIs (monoamine oxidase inhibitors)
- β2-blockers
- Corticosteroids
- Diuretics
- Muscle relaxants
- Sympathomimetic xanthine derivatives
Contraindications
Hepatic impairment, liver cirrhosis, severely impaired liver function
Side Effects
- Fatigue
- Nausea
- Palpitation
- Headache
- Dizziness
- Tremor
Pregnancy & Lactation
Caution recommended during first trimester of pregnancy. Caution recommended if administered to breastfeeding patients due to potential passage into breast milk. Terbutalin passes into breast milk but is unlikely to influence the child with therapeutic doses. Transient hypoglycemia reported in newborn pre-term infants after maternal β2 agonist treatment.
Precautions & Warnings
Dose should be reduced in patients with severely impaired renal function, genetic deficiencies of plasma cholinesterase, severe ischemic heart disease
Overdose Effects
No reported case of bambuterol overdosage. Overdosing may result in high levels of terbutaline and similar symptoms, including headache, anxiety, tremor, nausea, tonic muscle cramps, palpitations, tachycardia, cardiac arrhythmias, fall in blood pressure, hyperglycemia, and lactacidosis may occur. High doses may cause hypokalemia due to potassium redistribution.
Therapeutic Class
Short-acting selective & β2-adrenoceptor stimulants
Storage Conditions
Keep below 30°C, away from light & moisture, out of the reach of children
