Indications
- Invasive aspergillosis
- Candidemia (nonneutropenic) and disseminated candidiasis
- Esophageal candidiasis
- Serious infections caused by Scedosporium apiospermum and Fusarium Species
- Patients intolerant of, or refractory to other therapy
Pharmacology
Voriconazole is a triazole antifungal medication that inhibits ergosterol synthesis by inhibiting CYP450-dependent 14-alpha sterol demethylase, resulting in a depletion of ergosterol in fungal cell membrane
Dosage & Administration
- Oral tablet and powder for suspension to be taken at least one hour before or one hour following a meal
- Loading/maintenance dose regimen based on body weight for oral intake and specific fungal infections for injection
- Dosage varies for different fungal infections
Interaction
- Requires dosage adjustment and monitoring when co-administered with CYP3A4, CYP2C9, and CYP2C19 inhibitors and inducers
- May increase concentrations and activity of drugs that are CYP3A4, CYP2C9, and CYP2C19 substrates, necessitating dose reduction and adverse reaction monitoring
- Requires increased maintenance oral and intravenous dosage with co-administration of specific drugs
Contraindications
- Known hypersensitivity to Voriconazole or any other components of this drug
- Co-administration with specific drugs due to risk of serious adverse reactions or loss of efficacy

Pregnancy & Lactation
No adequate and well-controlled studies in pregnant women; use during pregnancy only if potential benefit justifies potential risk to the fetus

Long-term exposure (>180 days) requires careful assessment of benefit-risk balance; association with squamous cell carcinoma of the skin reported
Use in Special Populations
Efficacy not established for children under 12 years of age
