Indications

  • Invasive aspergillosis
  • Candidemia
  • Disseminated candidiasis

Pharmacology

Inhibits ergosterol synthesis by inhibiting CYP450-dependent 14-alpha sterol demethylase

Dosage & Administration

  • Oral:
    • To be taken at least 1 hour before or after a meal
    • Dosage varies based on body weight
  • Injection:
    • Invasive Aspergillosis:
      • Loading dose: 6 mg/kg 12 hourly for the first 24 hours
      • Maintenance Dose: 4 mg/kg 12 hourly
    • Candidemia in nonneutropenic patients:
      • Loading dose: 6 mg/kg 12 hourly for the first 24 hours
      • Maintenance Dose: 3-4 mg/kg 12 hourly
    • Scedosporiosis and Fusariosis:
      • Loading dose: 6 mg/kg 12 hourly for the first 24 hours
      • Maintenance Dose: 4 mg/kg 12 hourly

Interaction

  • Adjust dosage with CYP3A4, CYP2C9, and CYP2C19 inhibitors or inducers
  • May increase concentrations and activity of other drugs metabolized by CYP3A4, CYP2C9, and CYP2C19
  • Increase maintenance dosage with co-administration of Phenytoin or Efavirenz

Contraindications

  • Known hypersensitivity to Voriconazole:
    • Co-administration with specific drugs due to risk of serious adverse reactions
    • Co-administration with specified drugs due to risk of loss of efficacy
