Indications
- Adjunct to diet and exercise
- Improve glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Linagliptin inhibits DPP-4 enzyme
- Increases concentrations of active incretin hormones
- Stimulates insulin release from beta cells
- Decreases glucagon secretion from alpha cells
- Metformin lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Individualized based on effectiveness and tolerability
- Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
- Gradual dose escalation to reduce gastrointestinal side effects
Interaction
- Cationic drugs may interact with Metformin
- Carbonic Anhydrase Inhibitors may induce metabolic acidosis
- Inducers of P-glycoprotein and CYP3A4 Enzymes may reduce Linagliptin efficacy
- Drugs affecting glycemic control may lead to loss of control
Contraindications
- Renal impairment
- Metabolic acidosis (diabetic ketoacidosis)
- Hypersensitivity to Linagliptin or Metformin Hydrochloride

- Nasopharyngitis
- Diarrhea
- Hypoglycemia
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Exercise caution during lactation
Precautions & Warnings
- Discontinue in case of lactic acidosis or pancreatitis
- Temporarily discontinue before radiologic studies or surgical procedures
- Monitor Vitamin B12 levels
- Be cautious with alcohol and other medicines
