Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Linagliptin inhibits DPP-4 enzyme
- Increases concentrations of active incretin hormones
- Stimulates insulin release from pancreatic beta cells
- Decreases secretion of glucagon from pancreatic alpha cells
- Metformin Hydrochloride lowers plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Individualized dosage based on effectiveness and tolerability
- Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
- Gradual dose escalation to reduce gastrointestinal side effects
- Dose adjustment when switching between immediate release and extended release tablets
Interaction
- Potential interaction with cationic drugs
- Risk of metabolic acidosis with carbonic anhydrase inhibitors
- Effect of inducers of P-glycoprotein and CYP3A4 enzymes on Linagliptin exposure
- Effect of drugs affecting glycemic control
Contraindications
- Contraindicated in patients with renal impairment
- Contraindicated in acute or chronic metabolic acidosis
- Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride
Side Effects
- Most common side effects: nasopharyngitis and diarrhea
- Increased risk of hypoglycemia when combined with sulfonylureas
Pregnancy & Lactation
- Limited studies in pregnant women, should be used during pregnancy only if clearly needed
- Caution in administration to lactating mothers
Precautions & Warnings
- Discontinue Metformin in case of lactic acidosis
- Promptly discontinue in case of suspected pancreatitis
- Temporarily discontinue during radiologic studies and surgical procedures
- Monitor Vitamin B12 levels annually due to Metformin's impact
- Caution in conditions leading to lactic acidosis
- Instructions regarding alcohol, other medicines, counseling, travel and emergency situations
