Indications
- Adjunct to diet and exercise
- Improves glycemic control in adults with type 2 diabetes mellitus
- Appropriate when treatment with Linagliptin and Metformin Hydrochloride is necessary
Pharmacology
- Linagliptin is a DPP-4 inhibitor
- Increases active incretin hormones
- Stimulates insulin release and decreases glucagon secretion
- Metformin is a biguanide antihyperglycemic drug
- Lowers plasma glucose levels
- Improves insulin sensitivity and utilization
Dosage & Administration
- Individualized dosage based on effectiveness and tolerability
- Maximum recommended dose of 2.5mg Linagliptin and 1000mg Metformin twice daily with meals
- Gradual dose escalation recommended to reduce GI side effects
Interactions
- Potential interactions with cationic drugs
- Interaction with carbonic anhydrase inhibitors may induce metabolic acidosis
- Inducers of P-glycoprotein and CYP3A4 enzymes may reduce Linagliptin efficacy
- Certain drugs affect glycemic control and may lead to hyperglycemia
Contraindications
- Contraindicated in patients with renal impairment
- Contraindicated in acute or chronic metabolic acidosis
- Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride
Side Effects
- Most common side effects are nasopharyngitis and diarrhea
- Hypoglycemia more common with Linaglip-M and sulfonylureas
Pregnancy & Lactation
- No adequate studies in pregnant women
- Use during pregnancy only if clearly needed
- Caution in lactating mothers
Precautions & Warnings
- Discontinue in case of lactic acidosis
- Monitor vitamin B12 levels annually
- Alcohol consumption may cause low blood sugar
- Family members should be aware of side effects
Overdose Effects
- Supportive measures in case of overdose
- Removal of unabsorbed material
- Hemodialysis or peritoneal dialysis unlikely for Linagliptin
- Metformin dialyzable
- No dose-related adverse reactions for Linagliptin up to 600mg
- Metformin overdose may lead to hypoglycemia and lactic acidosis
