Indications
- Improves glycemic control in adults with type 2 diabetes mellitus
- Adjunct to diet and exercise
- Appropriate for treatment with both Linagliptin and Metformin Hydrochloride
Pharmacology
- Linagliptin inhibits DPP-4 enzyme
- Increases concentrations of active incretin hormones
- Stimulates insulin release from pancreatic beta cells
- Decreases glucagon secretion from pancreatic alpha cells
- Metformin lowers basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Individualized dosage based on effectiveness and tolerability
- Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
- Gradual dose escalation to reduce gastrointestinal side effects
- Recommended starting dose for patients not treated with Metformin Hydrochloride
- Switching from individual components to combination containing same doses
Interaction
- Cationic drugs competition for renal tubular transport systems
- Carbonic anhydrase inhibitors inducing non-anion gap, hyperchloremic metabolic acidosis
- Rifampin decreasing Linagliptin exposure
- Drugs affecting glycemic control leading to loss of glycemic control
Contraindications
- Contraindicated in patients with renal impairment
- Acute or chronic metabolic acidosis (diabetic ketoacidosis)
- Hypersensitivity to Linagliptin or Metformin Hydrochloride
Side Effects
- Most common side effects: nasopharyngitis and diarrhea
- Hypoglycemia more common in patients treated with Traneta M and sulfonylureas
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Caution during lactation
Precautions & Warnings
- Discontinue in case of lactic acidosis or suspected pancreatitis
- Temporary discontinuation for radiologic studies and surgical procedures
- Monitoring of Vitamin B12 levels due to potential decrease
- Advisory on alcohol consumption and other medicines
- Precautions in travel and emergency preparedness
Overdose Effects
- Supportive measures in case of overdose
- Hemodialysis or peritoneal dialysis for Metformin Hydrochloride
- No dose-related clinical adverse drug reactions up to 600 mg of Linagliptin
- Hypoglycemia and lactic acidosis reported in Metformin Hydrochloride overdose
