Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Inhibitor of DPP-4 (dipeptidyl peptidase-4)
- Increases concentrations of active incretin hormones
Dosage & Administration
- Dosage should be individualized based on effectiveness and tolerability
- Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
- Dose escalation should be gradual to reduce gastrointestinal side effects
Interaction
- Potential interaction with cationic drugs eliminated by renal tubular secretion
- Concomitant use with carbonic anhydrase inhibitors may induce metabolic acidosis
- Rifampin decreased Linagliptin exposure, suggesting reduced efficacy when administered with a strong P-gp or CYP 3A4 inducer
- Certain drugs tend to produce hyperglycemia and may lead to loss of glycemic control
Contraindications
- Renal impairment
- Acute or chronic metabolic acidosis (diabetic ketoacidosis)
- Hypersensitivity to Linagliptin or Metformin Hydrochloride
Side Effects
- Most common: nasopharyngitis and diarrhea
- Hypoglycemia is more common when used with sulfonylureas
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Caution when administered to a lactating mother
Precautions & Warnings
- Discontinue in case of lactic acidosis or suspected pancreatitis
- Monitor Vitamin B12 levels due to potential Metformin-related decrease
- Follow instructions regarding alcohol, other medicines, counseling, travel, and emergency situations
- Potential to cause hypoglycemia
Overdose Effects
- Employ usual supportive measures in case of overdose
- Hemodialysis or peritoneal dialysis unlikely to remove Linagliptin but Metformin Hydrochloride is dialyzable
- No dose-related clinical adverse drug reactions with Linagliptin up to 600 mg
- Hypoglycemia reported in approximately 10% of Metformin Hydrochloride overdose cases
