Indications
- Adjunct to diet and exercise for improving glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Inhibitor of DPP-4 enzyme (Linagliptin)
- Biguanide type oral antihyperglycemic drug (Metformin Hydrochloride)
Dosage & Administration
- Individualized dosage based on effectiveness and tolerability
- Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
- Gradual dose escalation to reduce gastrointestinal side effects
- Different starting doses for patients currently treated and not treated with Metformin Hydrochloride
- Extended-release tablet available with individualized dosage not exceeding the maximum recommended total daily dose
Interaction
- Potential interaction with cationic drugs
- Possible induction of metabolic acidosis with carbonic anhydrase inhibitors
- Decreased Linagliptin exposure with inducers of P-glycoprotein and CYP3A4 enzymes
- Effect on glycemic control by certain drugs
Contraindications
- Contraindicated in patients with renal impairment, acute or chronic metabolic acidosis, and hypersensitivity to Linagliptin or Metformin Hydrochloride
Side Effects
- Common side effects include nasopharyngitis and diarrhea
- Increased risk of hypoglycemia in patients treated with Linatin M and sulfonylureas
Pregnancy & Lactation
- Caution advised for use during pregnancy and lactation due to lack of adequate studies
Precautions & Warnings
- Discontinue in case of lactic acidosis or suspected pancreatitis
- Temporary discontinuation for radiologic studies, surgical procedures, or intravascular administration of iodinated contrast materials
- Monitoring of Vitamin B12 levels and hematologic parameters
- Warning signs and precautions for lactic acidosis, hypoglycemia, and pancreatitis
- Guidance on alcohol consumption, other medicines, counseling, travel, and emergency preparedness
