Function

  • Improves glycemic control in type 2 diabetes mellitus

  • Decreases hepatic glucose production

Dosage and Administration

  • Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily
  • Recommended starting dose: 2.5 mg Linagliptin and 500 mg Metformin Hydrochloride twice daily for patients not previously treated with Metformin
  • Dose escalation should be gradual
  • Dosage should be individualized based on effectiveness and tolerability
  • Extended-release tablet should not exceed total daily dose of Linagliptin 5 mg and Metformin Hydrochloride 2000 mg
  • Different starting doses for patients already treated with Metformin and/or Linagliptin

Interaction

  • Potential interaction with cationic drugs eliminated by renal tubular secretion
  • Concomitant use with carbonic anhydrase inhibitors may induce metabolic acidosis
  • Rifampin decreased Linagliptin exposure
  • Certain drugs may affect glycemic control

Contraindications

  • Contraindicated in renal impairment
  • Contraindicated in acute or chronic metabolic acidosis
  • Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride

Side Effects

  • Most common side effects: nasopharyngitis and diarrhea
  • Hypoglycemia more common in patients treated with sulfonylureas

Pregnancy and Lactation

  • Should be used during pregnancy only if clearly needed
  • Caution should be exercised when administered to a lactating mother

Precautions and Warnings

  • Discontinue in case of lactic acidosis or suspected pancreatitis
  • Temporarily discontinue in certain medical procedures
  • Metformin may lower Vitamin B12 levels
  • Symptoms and precautions for lactic acidosis
  • Precautions for alcohol, other medicines, counseling, travel, and emergencies
  • May cause hypoglycemia

Overdose Effects

  • Usual supportive measures should be employed
  • Linagliptin is unlikely to be removed by dialysis
  • Metformin Hydrochloride is dialyzable
  • No dose-related clinical adverse drug reactions up to 600 mg of Linagliptin
  • Reported hypoglycemia and lactic acidosis in Metformin Hydrochloride overdose cases
