Indications
- Treatment of hypertension
- Renal protection in type-2 diabetic patients with proteinuria
Pharmacology
- First non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- 25 mg twice daily recommended if 50 mg once daily is inadequate
- Total daily dose ranges from 25 mg to 100 mg
- Different dosage recommendations for different patient groups
Interaction
- Rifampicin and fluconazole reduce levels of active metabolite
- Concomitant use with hydrochlorothiazide may potentiate antihypertensive effects
- Potential interaction with potassium-sparing diuretics and potassium supplements
- Attenuation of antihypertensive effect by indomethacin
- Increased risk of renal impairment with certain drug combinations
Contraindications
- Contraindicated in pregnant women
- Hypersensitivity to any component
- Should not be administered with Aliskiren in diabetic patients
Side Effects
- Mild and transient side effects
- Common side effects include dizziness, diarrhea, nasal congestion, cough, upper respiratory infection

Pregnancy & Lactation
- Pregnancy Category D
- Risk increases if administered during second or third trimesters
- Unknown excretion in human milk
- Consideration for discontinuing nursing or drug based on importance to the mother
Precautions & Warnings
- Reduced fetal renal function and increased morbidity and death in second and third trimesters
- Risk of symptomatic hypotension in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
- Changes in renal function reported in renal impaired patients
Use in Special Populations
- No initial dosage adjustment necessary in patients with mild renal impairment
- Lower starting dose recommended for moderate to severe renal impairment or patients on dialysis
- Low dose consideration for patients with history of hepatic impairment
